24-hour mental health service plan system offered to adults with serious mental health concerns Client-centered voluntary service * crisis response, case management and supportive housing with a team of professional staff trained in social work, mental health nursing and occupational therapy Case Management - Community-based support for people with a serious mental illness * based on personal need and client directed * goals of case management are to assist people to live independently, assist people in making their own decisions, and to assist people in accessing resources * life skills training on a one-on-one basis or within a small group * information, assistance and encouragement are provided Case management service is provided in-home or a place of choice within the community |
