Service Description:	Assessment, development and implementation of rehabilitation and recovery plans * focus on daily living and housing, health and wellness, relationships, finances, access to employment and education and community involvement   Transitional Case Management Team - Facilitates hospital discharges for people who need community-based support * creates and implements plans for enhancing stability, connecting with services and meeting urgent needs * assists with short-term crisis management * information and referrals * conducts assessments for skills, functional ability, needs, and barriers * clients identify personal goals and preferences to work toward recovery * helps clients navigate mental health system and barriers * advocacy to obtain services and supports
