Service Description: Comprehensive and flexible community withdrawal management, addiction and mental health crisis care * services provided through the Ontario Telemedicine Network and telephone by registered nurses

Nurse Practitioner Services - primary care for people experiencing mental health and/or addictions and who are without a regular primary care provider or have an inability to access their primary care provider * services provided through various points of access, including the Ontario Telemedicine Network * email for NP Service: [email protected]

Eligibility / Population(s) Served: Clients 16 years and older * must be willing to enter program and not exhibiting aggressive and/or violent behaviours
TWCSP: Must be accompanied by support person and able to physically attend identified OTN site

TWCSP Community Withdrawal Management: must be willing to reduce use of one or more substances, deemed safe to withdraw in the community, have a safe and supportive withdrawal/recovery environment, meet provincial standards or withdrawal management

TWCSP Crisis Support: must be experiencing crisis related to addiction or mental health concerns, not appropriate if client is in crisis related to housing
