Health promotion, disease and injury prevention and chronic disease management to clients Works to promote overall health and well-being and improve access to health care and psychiatric services in the community * group and one-on-one services Diabetes Screening Program - Early detection and screening for diabetes with follow-up with appropriate health care professionals Diabetes Support Clinics - Monthly clinics for those who have pre-diabetes or diabetes to assist in the implementation of healthier lifestyle changes Foot Care Clinics - Assessment, treatment, and education for those at risk of developing foot problems Occupational Therapy - Helps people to succeed at activities they want or need to do * person, activity and environment are assessed * one-on-one and group interventions may be directed at making adjustments |
