Addiction Transitional Case Management Service - Assessment and Referral
- Program facilitates discharge process from London Health Sciences Centre and the Stabilization Space * ensures patients have linkages to appropriate addiction and mental health services and supports upon discharge * patient works with Addiction Transitional Case Manager to create and implement plan for care

- Offers housing opportunities as a part of addiction treatment planning

Back On Track Program
- Provincially mandated program for convicted impaired drivers and drivers charged with two or more roadside suspensions

Community Opioid Addiction Program
- Offers direct client services and community education for people with opioid addictions * assessment and screening required for entry into the treatment program

French Mental Health and Addiction System Navigation Program - Case Management
- Provides access to mental health and/or addiction support services in French
